Issues with Mail Merge options

I can not Insert Mail Merge Field to work properly.  It will not let me setup mailing label.  I have no choices.  I only have choice of Last Name First Name Address 1 City.  I cannot rearrange or add state and zip.  With agents, I have uninstalled and reinstalled my Word software twice without correcting issues.

@Betsy2010 Can you upload a copy of the mail merge main document and the data source so that we can investigate the problem.
