Footnote Numbering Incorrect

I have a 50-page word document where the footnotes go 1, 2, 3, 4, 5, 1, 7, 8 ......

 

If I delete the second "1" footnote and put in a new one, it is "1"

 

If I try to insert a new footnote in this general area, it comes out "1" if before or "2" after.

I have tried convert to endnotes and convert to footnotes

 

There are no section breaks in the document. 

 

I've tried copy and past to a new file. I get the same thing.

How can I fix this?

You can share a sample document so that I, or some other contributor in the forum, can have a closer look at the sample.

The decision is yours, obviously. However, note that nobody here needs to see any of your actual content. You can make a copy where you add som "dummy" text and leave a couple of footnotes (also with dummy text) for illustration of the issue. 

@Big_Jim_Slade We would want to see a couple of pages with properly numbered footnotes and at least the page with the problem one. It need have no meaningful content. Here is how to remove confidential or proprietary content in a sample and post a link.
