Who decided that close all windows should leave one hidden?

I find it frustrating when I use the close all windows option and discover that one window is still hiding somewhere, forcing me to hunt it down manually. Who decided this behavior made sense, and why isn't there a truly universal command that closes every open window without leaving exceptions behind? I'd love to know whether this is intentional design, an operating system limitation, or simply an overlooked usability issue.
