How to capture the comments from the approval request and display it inthe share point list?

Hi Everyone,

 

i am looking for support to get information on , how to capture the comments received from two approvers and display it in the share point list. But i wanted to only have both their approval decisions captured in one column in my sharepoint list.

 

Please share your input?

    I need the comments message to be displayed in the sharepoint list in comments column.
