Every time I flag an email in Outlook, it immediately checks if off instead of leaving the flag. I've gone through the flagged settings and can't find anything to change it back.
I've tried the following:
Created a new OST
Repaired office
Ran Outlook.exe /cleanviews and cleanreminders
Removed the flag and re-added it via Outlook

Having the same issue. Flagging an email and the flag immediately disappears.

I was unable to reproduce your issue. However, based on research, the flag disappearing act has been happening for a long time now without a fix. Here are several posts that you may find interesting.
