Pulling my hair out trying to register Yealkink SIP-T48S

I decided to try Skype for Business for an office phone and I am regretting that decision. The best phone appeared to be the T48S because it had the firmware and was made for Skype for Business. 

 

So I purchased the phone, and I cannot find instructions or get competent support to connect it. I) have plumbed the depths of Google looking for instructions and opened two separate tickets.  The support people who called me back had no idea.  This is worse than the Cisco phone I used to have.

 

The phone is connected to the network. That part is fine.  But it will not register, and I have no idea why. I am attaching a screen shot of my registration page.  

 

Can anyone tell me what I am doing wrong?

I assume you've checked all the basics.
You have an E5 license and a phone number provisioned?
You've checked your firewall logs to make sure it is not being blocked somehow.
Does your account require MFA? Do you have CA setup? Check the auth logs in Azure or ADFS to make sure the device is not being denied auth.
