Multiple Recipients

I created several forms.  I send the form link in a mass email.  One of 2 things happen.  The form will only work for one person.  ( I have it set for Any can respond to this form).  Or the recipient fills out the form, but the answers become the heading. I have watch several videos, read many forums.  Please advise.
