Checklist creation
I would like to convert my current excel form to Microsoft Form, can some one suggest me how do it since I am new to Forms.
Added pic for reference.

sunilkumar2245 you would have to that manually in Forms, there is no way to import that excel sheet into Forms. Alternativelya better way might be to look at a PowerApps form linked to a SharePoint list as the data source/repository. That way you can have checkboxes and the form can replicate your current one.
