Hello,

I made a survey with the first question being the "name". However the name comes attatched with the answers in the excel.

 

Is it possible to do an anonymous form but keep an independent record with the name of the people who answered it?

@Andr_Cecliono it isn't. If you have a question in the form for the responder's name that will always be in the spreadsheet. If you want an independent record of the responses without the name then you will need to build a flow in Power Automate to save the responses minus the name to a list in SharePoint.
