Checkboxes Broken When I Copy Sheet

I have a simple little worksheet I'm using to help me with keeping tabs on inventory. I have some checkboxes on this worksheet that help me visualize whether or not I need to order something with some conditional formatting. I'm trying to use this as a sort of weekly checklist, and, as such, I would like to be able to have a blank template sheet so that I can copy it for each passing week. When I go to copy the completed sheet, some of the checkboxes that I'm using seem to break. Most of them copy over just fine, but some show text that says: "The picture can not be displayed." Others are simply gone. The interesting part of this, though, is that those that are "broken" still function as normal (e.g. when you click on them, it still changes the state of the cell from TRUE to FALSE). That is, until you save the file and try to open it again. When I do that, Excel comes up with a warning that states: "We found a problem with some content in 'Weekly Inventory Check.xlsx'. Do you want us to try to recover as much as we can? If you trust the source of this workbook, click Yes." Being that I created the workbook, I click yes, and a dialogue box appears (see below). It then comes back up with this workbook, but the "broken" checkboxes are gone (only the "broken" ones). I've dug around on forums both here and elsewhere, but I can't seem to find anything that works. I've tried going into the options and selecting "Optimize for compatibility" under User Interface Options in General, I've tried selecting and deselecting "Cut, copy, and sort inserted objects with their parent cells" under "Cut, copy, and paste" in Advanced. I'm not sure what's happening here, but I would greatly appreciate some insights as to why this might be happening. Below I will include some screenshots of what's going on:

Above is the worksheet that I copied from the one that works correctly. As you can see, some of the boxes are "broken."

I'm afraid controls often misbehave like this and the only way I know to fix it is by deleting them and adding them back in.
