Hi Mark,
I suspect that the problem is that the Navigation Pane is not seeing your headings as headings.
See my article: Headings that do not show up in the Navigation Pane or in a Table of Contents (or in Cross-References / Hyperlinks). This has to do with the paragraph style applied to a heading having an Outline level.
