Macros

The anti-spam system reports that there is an executable macro in the file. But there is nothing in the list of macros. Although if you change the doc extension to 7z and open it as an archive, you really have a Macros folder. Question: how can I see what kind of macro it is and what it does?

The file may have contained a macro at one time that was later deleted. Inserting an ActiveX control in the document may also create a macro storage area, even if you never write code for it. The security check doesn't know or care whether there's an actual macro there, only that there is a macro storage area in the file.
