Duplications in the list of figures!?

Hi all,

I have, as usual, labeled my figures using right-click - Insert label, and then created a list of figures.
As you can see, all of a sudden some figures show up double and triple, some labels (like #7 here) are simply skipped, while yet other numbers are not chronological or duplicated.

Update all fields also didn't help, neither did deleting and reinserting all captions. 

What the hell is going on here?! And how can I fix this mess?
I'm using Word on Mac if that helps.
I would be insanely grateful to you guys if someone could point me in the right direction.
