How about this:
- Enable the Developer Tab:
- Go to File > Options > Customize Ribbon.
- Check the box next to Developer and click OK.
- Insert Checkboxes:
- In the Developer tab, click on Check Box Content Control.
- Place the checkbox where you want it in your checklist.
- Add Text for Pass/Fail:
- After inserting the checkbox, type "Pass" or "Fail" next to it as needed.
- Format the Checklist:
- Adjust the alignment and spacing to make the checklist visually appealing.
- Save as PDF:
- Go to File > Save As and choose PDF as the file format.
- Ensure that the checkboxes remain interactive in the PDF.

Thanks for the suggestion. I just tried your suggestion and yes that does put the checkboxes at the beginning of the line similar to what I had posted. I'm trying to figure out if there is a way to do it as a bulleted list (or something else) so that as I add lines the pass/fail is automatically added.
