Unable to Format SDXC Card

I have a SDDXC Card that will not format.   Windows Disk Management shows a healthy primary partition but I am unable to format or delete the partition.   When I use DiskPart, I can clean the disk but the partition does not delete.  I cannot create a new partition because the single old partition takes up all space on the disk. 

Also, when I use diskpart, I cannot select the primary partition
