Jotform’s Fillable PDF Form Creator is a web-based option if you prefer arranging fields visually without installing software. It has a free plan with limits, while higher usage requires a paid plan.

  1. Open the Fillable PDF Form Creator and upload your existing PDF, or start with a template.
  2. Add text fields for names, email addresses and phone numbers.
  3. Add checkboxes for choices and a multiline field for additional comments.
  4. Adjust the field positions and sizes, then download the fillable PDF.

Make sure you download the fillable PDF, rather than only sharing a link to an online form. Recipients can enter and save their answers using a compatible free PDF reader.

This is convenient if you already designed the page in Word: export it as a PDF first, then add the fields in Jotform. Since it’s an online tool, you’ll need to upload the document.
