Clear the Quick Access cache.
Corrupted cache files can make your pins vanish. To rebuild them:
del /F /Q %APPDATA%\Microsoft\Windows\Recent\*
del /F /Q %APPDATA%\Microsoft\Windows\Recent\AutomaticDestinations\*
del /F /Q %APPDATA%\Microsoft\Windows\Recent\CustomDestinations\*
