Whose idea was it to make Restart and Update and Shut Down look identical?

Why on earth would someone design Restart, Update and Shut Down to look virtually identical in the power menu? I just want to know whose bright idea it was to make three options with completely different outcomes so easy to confuse, because one wrong click can cost me my unsaved work.
