Thunderbolt controller crashing the entire PCIe bus

My Thunderbolt controller keeps crashing and taking down the entire PCIe bus with it, causing all my connected devices to disconnect randomly during use. This happens consistently when I plug in or unplug Thunderbolt devices, and I'm trying to figure out if this is a driver issue, a firmware problem, or a hardware defect with the controller itself.
