The clock in Windows 10 displays the time with a delay; it only updates after a refresh.
The system clock in the lower-right corner of the taskbar sometimes stops updating and displays a time that is a few minutes or even longer behind the actual time. When you click the clock to open the calendar panel, the time immediately updates to the correct time, but after a while, it falls behind again, requiring manual synchronization or a restart to return to normal.

The battery on your motherboard that powers the system clock when the PC is off might be failing. If it's drained, the time will be incorrect on startup. Replacing this battery is a common fix.
If the clock consistently falls behind even after a restart, it could be a sign that the small CMOS battery on your motherboard is running low. This battery keeps the system clock running when the PC is off. If it's dying, the clock might lose time during shutdowns and gradually drift after startup.
Hi Max, if clicking the clock refreshes it, Windows time is probably correct and the taskbar display is lagging. Restart Explorer, check for pending Windows updates, and run sfc plus DISM if it repeats. I would also update graphics drivers, because taskbar redraw problems can show up as a frozen or delayed clock.
