Your graphics card might have a physical or architectural limit on the number of monitors it can support using certain connection types. Some cards, for example, can only use two "legacy" connections (like HDMI or DVI) simultaneously and require an active DisplayPort adapter for the third screen. A standard passive adapter will not work.
