PowerApps: Error Implementing Form onto PowerBI

Hello,

I am trying to implement an app I created onto a PowerBI report. The app works perfectly on the desktop editor and in play mode. However, when I add the app onto the PowerBI report associated with it, it does not come in correctly. I am selecting all fields included in the app on the PowerBI side when adding in the app. I have also included a second screenshot showing what the homepage display should look like. Any help is appreciated, thank you!
