JaWa1478 When you use "Set content approval status" action, you have to pass the updated ETag to the action.
Before this action, get the latest ETag value from SharePoint library using "Get File properties" action first and then the "Get file Metadata" as mentioned in the comments of this post: Renaming filename and set content approval status - power automate
