A couple of options to consider:
1. You could use PowerApps for a more custom experience. You can find the customise option when you click + New in the list and then the top right of the open right side pane will have the option.
2. Alternatively if this is an internal form to your business, you could use Microsoft Forms which will force users to enter particular data and allows for the uploading of a file, before submission.
When the form is submitted, the flow can upload that data and attachment to the list.
Hope something in there gives you an idea.

Considering I'm not a developer, is there some tutorial or step-by-step instruction I can follow? I could create a form but, after creation, I do not know what to do with it. How do I link to the "Upload" command in my Document Library? How do I actually perform the upload?
