Planner and Loop integration

I have recently started to use Loop and really want to learn how to integrate Action Items from loop into Planner projects in my Teams Workspaces. I have tried to figure this out and while I know the action items are added to planner and individual to dos, I can't figure out how to connect it to a Planner project for an individual Teams Workspace.
