I was able to fix this issue by deleting duplicated rules associated with old emails (Display names).
The only method to achieve this is by going into the user's Outlook Web App (OWA) and delete duplicate rules. Some users have to change their Outlook or GAL display names and that old email will corrupt the Rules structure.
Outlook Web App (OWA) > Settings > Options > Inbox and sweep rules
