Adaptive Card to gets survey from Users

I am trying to implement some survey app for outlook connector similar to below screen.
where i used Column set , input toggle for number.... when a user selects it and clicks on submit feedback should get saved . at the time of submit i am getting 401 error.
it would be great if someone can help me on this.
i am suspecting there is issue in body schema it is resulting null value.
I could some how able to finish the app . while testing It is working in Outlook web but not in outlook desktop.
AC version is 1.0 for outlook connector i am using
Can some one share your thoughts ASAP .
