Daily inspection report

I have an inspection form that field hands are filling out on paper right now, and I'm trying to automate it. I thought forms would be the right option, but it doesn't seem to have any customization for the look of things. 

The form is relatively simple. It should have a text field for name, date, and location. The rest would just be 3 radio buttons. 
"Is this task complete? Yes/No/NA"

Is there a way I can have a form template that gets filled out once a day and submitted through teams, or saved to onedrive?
