Sorry something went wrong

Morning, 

Today whilst building a form through excel - 3 times ive tried. 

I got as far as entering the title of the form when 

I notice that the form struggles to save when its saying saving along the top banner - it doesnt show as saved. 

Please help, as I say, 3 times ive tried and each is the same outcome.
