Response Template

Hello,

I tried searching for any posts with this question but did not seem to find anything.

Is it possible to add a template to a question's response field when creating a form?

Example

Question Field:  Please tell us more about your current computer.

Response Field:

Brand:

Processor:

HD Size:

RAM:

I know we could break this out into 4 separate questions, but trying to understand if we have the ability to do this in one.
