RobElliott thanks for that. Below are the headers from the Spreadsheet. Users may have a long list of assets, and asking the question is forms 'what is name of the asset?' will only allow 1 response I assume.

Fionalamont you can't replicate that in Forms, it would need to be 1 form per asset.
