MULTIPLE ANSWER IN FORMS

Hi All,

I am having a hard time on how to populate excel columns from ms forms. I am just a newbie and trying to learn new.

Here's the scenario:
In question 1 i selected Choice 1 and Choice 2 and it must populate the excel table.

Question 1:

   Choice 1

   Choice 2

   Choice 3

   Choice 4

and it must store to excel:

Choice 1     CHoice 2    Choice 3    Choice 4

   True             True
