Microsoft Forms

I created a form for my students to register for classes and it worked good for a couple weeks. Today it would not work so I went to the Microsoft 365  and seen the following error on the top of my form...."This form can't be distributed as it is asking for personal or sensitive information.""

How do I resolve this issue?

There is a  Microsoft template called "registration and sign up forms". How is this different then what I am doing?
