Matrix style Q help

Hi

I am stuck and cannot figure it out, hoping someone can help.

How can I create 2 sections under a likert question style (Matrix style)? I have a list of options and for each, I need the responder to select 2 answers, 1 from each table.

here is the Q to give you more insight - 

Our school regularly communicates updates from/regarding;

1) Tick the extent to which you agree or disagree with each option (disagree, neutral, agree).
2) indicate how important each expectation is to you (not important, somewhat important, very important)
(So, there will be 2 responses needed for each question).

Alumni is an example of 1 of the drop downs.

Please help as I can't figure it out but I have done it in other surveys tools
