jmoore21 setting up a form is very easy, we do it almost daily at our company, and staff add their answers which are grabbed via a flow in Power Automate and saved into a list in SharePoint - we always do that as a backup and because Forms can be a bit flaky! What is it that you are finding difficult?

RobElliott how to get started... I have been playing with it. My supervisor said to use the main forms page. It gives an option of "text" or "choice". I can't find that anywhere. I go to teams page and open forms and i get a box to choose multiple choice, rank, cloud, etc. I clicked on multiple choice but i don't see what she is talking about. I'm sure it is just a simple click that i am missing.

jmoore21 you can't attach a form to an email, only provide a link to it using the </> code view button then adding html tags in the email body like this:
A simple example of saving a Forms response to a list in SharePoint is shown below. The trigger is when a new response is submitted, immediately followed by the first action get response details. Build this flow from blank, not from the template which has an error that Microsoft have never corrected which puts in a rogue apply to each which can cause problems with the flow running successfully.
