1 account, 2 computers (work and private) but missing a Form on one o them

Hi there, i've made a form on my private computer (mac) with my hotmail account. It's not visible anymore, but is still visible on my PC at work, with the same account, the form is still active , because people filled it in, and ik can open a link with the results (which is sent from my work pc) on my private computer. Hope someone can help me with this
