Customer Information Not Saving

I have noticed recently on several booking calendars that customer information is not saving properly.  We have Name, Address, Phone Number, and several custom fields marked as required in the booking.  The customers are completing all of the fields, and I can see those fields in the calendar in the booking.  The customer record is only capturing email address and phone number.  There is no name or address.  I also thought at one point, the custom fields were being attached to the customer record, and that it no longer happening.
