VLOOKUP over multiple columns

Using MS 365 Personal Excel on a MacBook. In learning VLOOKUP, I have had limit success for a conference registration template. I got the function VLOOKUP working for 5 columns OK. Now I'd like to skip 10 columns and use in columns 16 and 17. Is there someone who can take a look at my "dummy: spreadsheet Live and take control of my spreadsheet for 10 minutes
