This will do it. I prefer using named items for readability and not the long structured table references:

=LET(
    swap, IF(Counts, Header & " (" & Counts & ")", ""),
    joined, BYROW(swap, LAMBDA(v, TEXTJOIN(", ", 1, v))),
    joined
)
