Countif and and or match or function

Hi I'm trying to take all of our employee's diversity information (similar to the example A) and transfer the data to example B). We have around 6,000-7,000 employees and I've tried several countif formulas, but my numbers don't match what is on example A when I audit. Please let me know if you have additional questions.
