Building a Graph From Variable Data Points

I've been given a preexisting template to work with, and I'm struggling to reconcile it with the graph I've been tasked to create.  I'm attempting to combine each pair of highlighted cells on the "Printable" sheet to populate the corresponding cells in "Data for Graph".  Is there any way to take the schedule as presented and automatically fill the cells being referenced by the graph, given values for the "FROM" column may change from day to day and week to week?
