Hi, if the CSV upload succeeds but activation fails, I would first check the token seed format, time drift, and whether the token is assigned to the correct user before activation. Hardware OATH tokens are very sensitive to the CSV fields and current code timing. It also helps to test with one token first and confirm the user is allowed to use OATH tokens under the authentication methods policy.
