Hi, 
Even after the free 12 month trial of Azure subscription, and if I were to cancel my subscription, would I still be able to use this offer for a lifetime?

Hi wizardofyas the base requirement is an Azure subscription, as long as you have that, you should be able to use the free offer. The Azure SQL Database free offer provides 10 free databases per subscription for the lifetime of that subscription, but it requires an active Azure subscription to remain valid.

Even after your 12-month free Azure trial ends, you can continue using the Azure SQL Database free offer as long as you keep your Azure subscription active (even if it's on the Pay-As-You-Go tier without additional charges for the free databases).
