Tasks and checklist not saving

Hello, I hope you can help. Our tasks are randomly not working. Sometimes we could add content to the notes and when you go back into it, its completely disappeared.  Also the checklists dont appear for some of our team, on certain tasks.  We use this system as a form of communication and to instruct the next steps for our clients, so missing information is detrimental. Does anyone know of a way to fix this?  Alternatively can anyone recommend a more reliable system for task notes/to-do's to be shared throughout a team.  Many thanks
