Power apps didn't show data when I used a form.

Please see the pic. The data was used for the gallery but it(Table1) can't be connected to the form. 

As you see below, the data can be shown in 3spots. Also, I already connected the data'Table1) to the gallery. i don't know why. Could you do me a favor?
