Microsoft Forms is now integrated with PowerPoint for Office 365 to provide educators an easy solution of inserting a quiz or an assessment into a PowerPoint presentation.

To get started, simply click the Forms icon in PowerPoint to create a new form/quiz or insert one you've already created.

Click “Insert” to embed a form or quiz into your current slide.

With the integration of Forms in PowerPoint, a teacher, for example, can add a quiz to his or her deck and share it out with students. Students can then fill out the quiz and submit their answers without leaving PowerPoint.

Please note that this feature is only available for PowerPoint Windows 32. Learn more about how to insert a form or quiz into PowerPoint.

If you want to provide feedback about this feature, we'd love to hear from you! Please go to the Microsoft Forms UserVoice site to offer suggestions or vote on ideas others have already submitted.

This is great and all, but when I preview the slideshow (with the form in it) on my Surface Go tablet, I lose the ability to progress further in the presentation, or even exit.

Copper Contributor

Is this being made available for PowerPoint Windows 64?

Microsoft

@Benjamin Dalby yes, available for PowerPoint Win 64.

Copper Contributor

On which version does it appear, as i'm not seeing it? Currently running 1803 (Build 9126.2295)

I'm guessing I need to speak to our IT department to update our version of Office.

Microsoft

@Benjamin Dalby You need to be on version 1807 (Build 16.0.10711.20012) or later to be able to see the entry point. For details please refer to this article.

I still don't have this.   Smiley Sad    I first thought that it may just take a few weeks to arrive.  But I'm still waiting

I have O365 Pro Plus  Version 1811 (Build 11029.20079 Click to Run)

I'm really keen to demonstrate this to my colleagues.

What can I do to enable this??

Microsoft

@Angus McPherson thanks for your feedback. I will follow up with a separate mail with you to debug this issue.

Copper Contributor

I had the Office 365 Pro plus - with Forms included in PowerPoint. I uninstalled it to re install a more recent build since my colleague (also with the Pro Plus version) has a more advance build (larger build number) - and now I don't have it in my PowerPoint in the newest version I installed.

Microsoft

@JoeMic Hi Joe, thanks for your feedback. Do you mind to help us to debug this issue by submitting this problem in PowerPoint client? I will send you the detailed instruction in a separate message.

Why I can't see Forms in Insert menu? I am using Microsoft Office 365 ProPlus Version 1811 (Build 11029.20079). Office 365 E3 subscribers are eligible for this feature, right? @Anqi Du

Iron Contributor

@null null

I had the same issue.   And I resolved it with Anqi's assistance.

All I had to do was sign out and sign back in.   So open Powerpoint and sign out.

"Why I can't see Forms in Insert menu? I am using Microsoft Office 365 ProPlus Version 1811 (Build 11029.20079). Office 365 E3 subscribers are eligible for this feature, right?" - Currently PowerPoint team is working on a bug fix related to this issue. For now, you can sign out and re-sign in to PowerPoint client to solve this problem. Please let me know if it doesn't work for you.

Copper Contributor

@Anqi DuGreat, the Forms menu showed up after I signed out and signed in again! Thank you for your advice, Anqi Du :)

Copper Contributor

Hi, I have the same problem and signing out and signing in again doesn't work for me.

Microsoft

@Ben_F Hi Ben, could you report this issue by clicking on the smiley face on the upper right corner of your PowerPoint client and be sure to check attach your logs and screenshot so that engineers from PPT team can help to identify the issue? If your version doesn't see smiley face icon, you can also search "help" in-app to trigger the report process.

Microsoft

@Ben_F Hi Ben, we have identified your issue. Seems you were not logging with your org ID. This feature is currently only available to Office 365 Education and Office 365 Commercialsubscribers with version 1807 (Build 16.0.10711.20012) or later of PowerPoint for Office 365 on a PC. Learn more.

Copper Contributor

Hello! I could use some help. I have the 2016 MSO (16.0.9126.2336) 32bit version of PowerPoint. Can I have Forms?

Copper Contributor

Any idea if Forms integration will ever make it to consumer 365 subscribers?  I'm using a non-corporate issue SurfaceGo to take notes in my client meetings... and I'd love to replace my handwritten notes with a structured-entry option like MS Forms.  Currently using Smart Forms 365... but the app is extremely unstable and doesn't integrate with anything.  Thanks!

Copper Contributor

Love this feature and plan to use it. What I would find even more valuable is being able to embed the Forms Results page in PowerPoint so that real-time surveys can be performs in a presentation dynamically showing the audience the outcome. Any thoughts or options?

Microsoft

@bmahoney Thanks a lot for your feedback. Embedding Forms results in PowerPoint is definitely a good scenario that we'd like to explore more on it. We've logged this ask in Microsoft Forms feature backlog and we may reach out to you to better understand your scenario when we work on this feature. We also suggest you to put this idea into Forms UserVoice so that other users can vote for this idea and we can better prioritize.

Microsoft

@mtsdawson Thanks for your feedback. This feature has not been enabled for Microsoft consumer users now. But we'd love to learn more about your detailed scenarios. I will send you a private message shortly.

Brass Contributor

Am I correct in assuming that if you are using the 64bit version of Office 365 that this feature is not available?

Copper Contributor

I will also like to have this functionality in 64b O365 for consumers

Copper Contributor

I have been unable to make the "Forms" icon in powerpoint appear. even after logging out and in. I believe I have Office 365 ProPlus for students, Version 1808. Is it possible to get forms access in powerpoint under my license?

Copper Contributor

The Form icon is not visible. Microsoft Office 365 ProPlus version 1808 64 bit

Copper Contributor

Forms wasn't visible for me for a long time, but signing out of Office and signing back in worked.

Copper Contributor

@Anqi Du When should this become available for 64 bit systems?

Copper Contributor

Hi @Stephanie Brantley, I have a 64-bit system and it now works for me. I had to sign out of Office and then sign back in again to trigger Forms to appear though.

Steel Contributor

Teachers love adding Forms in PowerPoint Recordings.
This support documentation show admin must "Let people in your organization go to the Office store" to get the Form to display.
In education this is usually turned off for good reason so students don't install add-ins that aren't supported by the school.
Usually you can just deploy add-ins from the Office store. However I can't find this as an add-in.
Does anyone know how to just install the add-in for Forms in PowerPoint?

Iron Contributor

@Anqi Du   @Philemon Burney

Hi Anqi,

A year ago forms in Powerpoint worked just fine.  But sadly since then my org has cracked down and blocked all possible Office Add-Ins.  Consequently teachers can't add forms to Powerpoint any more.   I have just contacted by org and am awaiting their response.   But like Philemon mentioned,  it's common in school environments for admins to place restrictions upon office add-ins.

I will seeking to get all MS published add-ins unblocked and perhaps a few others too.   But it would also be useful if the forms feature could included within powerpoint in a different way to permanently avoid this issue.

Copper Contributor

Hi @Anqi Du ; @Angus McPherson ; @Philemon Burney

We have the same issue in our company. We have O365 E3 licenses.

The Office store is not accessible to our end users for security reason.

We see well the "Forms" add-in in powerpoint and it starts but when we want to insert a new Form we get the message "Your sys admin has blocked add-ins from the Office store". We didn't find also a way to deploy it through the "O365 Centralized Deployment service". This powerpoint Form add-in is not available in the store. And I tried by using the XML manifest coming from another device, it didn't work also.

Has anyone a solution ?

Powerpoint Forms error.jpg

Steel Contributor

There is no specific add-in. You have to enable it add-ins for all users.
I also asked here and got a response https://twitter.com/PhilemonBurney/status/1179395365143027712?s=20

Iron Contributor

@Philemon Burney @Anqi Du @Angus McPherson @GOKTAN ACAR

I just submitted a support ticket on the same issue. If I get a response, I'll share it here, but I'm seeing the same issue. You're left with two choices:

  • Enable users to use Forms in PowerPoint, but give up all control over which add-ins are allowed, some of which pose regulatory/privacy/confidentiality issues, Or
  • You can't use Forms in PowerPoint, but you retain control over add-ins.

It doesn't make sense. You can selectively deploy things like FindTime. I don't understand why Forms is treated differently. Hopefully Support offers help.

Steel Contributor

Thanks @Ryan Helmer  Please keep us informed.

Copper Contributor

@Ryan Helmer @Philemon Burney  I had some exchange with Microsoft and they told me that they are investigating how they can enable it and hope to have a result in coming days. No possibility to open completely the Office Add-ins store to all users for same reasons as Ryan.

Copper Contributor

I work at a College and have the same issue, we cannot find the Office Add-in to enable it. I'll raise a support ticket and post back if we make any progress.

Copper Contributor

Same case for Office 365 Powerpoint. It doesn't make sense. You can selectively deploy things like FindTime. I  also don't understand why Forms is treated differently. Support suggested raising an idea via Uservoice.

https://office365.uservoice.com/forums/273493-office-365-admin/suggestions/40088218-giving-possibili...

Raised the Idea for Office 365 Admin. If you have the same situation, please vote the idea, maybe this will help to speed up the process.

Copper Contributor

This is the feedback I've had from by ticket, it's not good news I'm afraid :(

If the option "Let users access the Office store" is switched off it is by design to get the "Office 365 has been configured to prevent individual acquisition and execution of Office Store Add-ins" notification and not be able to use the Forms feature in PPT (Please see the Note in the "Insert an existing form or quiz" section here: https://support.microsoft.com/en-ie/office/insert-a-form-or-quiz-into-powerpoint-1a316f81-9ea7-4bc2-bda0-024c0d780df1?ui=en-us&rs=en-ie&ad=ie;

Support has already raised a design change request to the Product group, asking for the Forms feature in PPT to work despite Let users access the Office store being switched off; it was rejected with the following justification:

"Thank you for contacting Microsoft regarding your recent PowerPoint design change Request about being able to add Forms without turning on the organization to the Office Store. We have investigated this issue and determined that we must reject this request.
The current behavior is not a regression, and the root cause is related to the way the Forms is integrated in PowerPoint to allow for users to be able to sign in automatically using the Office credentials. While we are looking to continually improve the experience the potential change is not trivial. After much consideration, we feel that this is not a change appropriate for a sustained engineering release. As we improve the platform over the next few releases we will keep this feedback in mind and improve the current experience.
Thank you for bringing this issue to our attention and continuing to work with us on improving our products for customers worldwide. Again, we apologize for the inconvenience."

I'll push for fix via other channels but it's looking like we will not be able to use this functionality while we manage Office 365 add-ins :(

Copper Contributor

This is infuriating.  As a public school, we cannot allow unmanaged access to all Add-Ins without authorization and still be compliance with the law.  Why in the world would you design one of your own Add-Ins in such a way that it cannot be managed using your own enterprise management tools?

Copper Contributor

Just another bump.  An all or nothing approach is not practical. The admin should be able to enable a single add-on.

Copper Contributor

I was wondering if there is any change or solution for this. I cannot find any :(

Copper Contributor

Nope.

Brass Contributor

i have the add in but it does not load at all, also do doe not give an error message. simply a gray screen. anyone seen something like this and know how to troubleshoot ?

Daniel_Herrera_Germany_0-1596021860908.png

Copper Contributor

I'd like to insert a form in the middle of my slides, but when playing the slide show, I can't get from that form back to the slides.  Is there a way to accomplish this?

Steel Contributor

I needed to try this again for a different purpose and still couldn't get this working in an Enterprise licenced tenant. We tested out turning on the Business Apps (Free) option but that doesn't work.

I did manage to embed a form using the Microsoft Web Viewer add-in using the form share link. You can add this app to PowerPoint and then just paste in the URL and remove the https:// as this is already provided in the app.  Works fine in preview and presentation mode with only issue needing to zoom our with Ctrl- sometimes when switching screens.

Copper Contributor

Can you add results from a FORM to powerpoint.

Also have the results update dynamically, as more responses are received during the presentation.

Copper Contributor

only way is to add the following add-in "Microsoft Web Viewer" without opening the whole store

Brass Contributor

Same issue here - under pressure to deploy 3rd party products to plug this gap.

Iron Contributor

Woohoo! It looks like this issue will be resolved. Look for MC230157 in the Admin Center... can't find this on the roadmap, but they reference this UserVoice feedback.

Steel Contributor

Has anyone been able to get this to work in a Business Tenant using E3 64 bit licenses via Central Deployment?

Everything seems to load for the sidebar and pick the Form but when we click on the Insert button We get the;

ADD_IN ERROR

You don't have permission to use this add-in. Contact your system administrator to request access.

What did you search for in the Office Store?

Now Feb 1, 2021

I just tried again and it is still not working.  AND the MC230157 in the Admin Message Center is now gone.

No Updates to the User Voice either

Anyone know what is going on with this Forms in PowerPoint via Central Deployment?
