Fuzzy Lookup run issue in excel

Hello All,

i need a help to run fuzzy lookup application in my excel 2013. by taking a long remote session by your team some one but still not activate Add-Ins fuzzy look up.

can anybody help and tell me where is the problem? you can also access the remote session.
