This is a bit of a loaded question but I'll do my best to answer. Quality and stability are always our top priority. I understand that issues can cause a lot of frustration and that they can be very disruptive. In general, we are able to resolve most issue very quickly once we have sufficient information on them which generally requires customers to open support tickets, so please make sure that as you encounter issues to open a support ticket. There are a variety of reasons why issues may unfortunately make it to an end-user or organization so I can't specifically address any one specifically other than to say we have many processes in place to prevent this (and as noted, quickly correct in the unfortunate case where issues do make it out the door).
