SPFX ListViewCommand set extension to show custom forms

I have a requirement in Modern SharePoint online site. The requirement is custom form need to be shown when document library Upload or New button is clicked , the controls will fetch the data from Database . On clicking Save button the document has to be uploaded with metadata filled and the unique Id need to be generated . Also this solution should be dynamic that whenever new document library is created automatically the above custom feature need to be available by default.
