problem with json list formatting - header

I'm trying to format a header on a list with json. I'm a newbie to json but I need a code that will present two fields at the top. One is an image field (that contains our client logo) and the other is field that contains our Client name and for the life of me i cant figure it out. Below is a screen shot.
